New medical profiling process implemented

Wright-Patterson Air Force Base has been selected as one of the first bases to implement the new Air Force medical profiling process. 

Functional limitations are now listed on the new Air Force Form 469, duty limiting conditions report, and will be used to notify supervisors, unit deployment monitors and commanders of a member's physical limitations. 

Hard copies of the Air Force Form 469 will no longer be issued by the 88th Medical Group. Instead, members will receive the Air Force Form 469 from their commander or designated representative after being validated within the 88th MDG. If physical limitations listed on the Air Force Form 469 impact fitness testing or squadron physical training, then members should contact their unit fitness program manager for further guidance.
