Medical Flight Standards (MFS/FCI) Branch Policy
Before the FCI physical examination, all supporting documents maintained at the Detachment on cadets, i.e. entry DoDMERB or MEPS physical exam, WINGS (PPQ) worksheet, consults, etc., must be scanned by the Detachment, and submitted in Adobe pdf format by email to USAFSAM.Med.Flt.Scr@us.af.mil
no later than 30 days prior to the evaluation. If the cadet had corneal refractive surgery (PRK/LASEK/Epi-LASIK/LASIK), they must meet the one year post surgery waiting period. All pre/post optometric/ophthalmic examinations, along with the completed corneal refractive surgery checklist, must be sent 30-45 days prior to the FCI appointment.
: Any additional documentation that MFS Staff have requested after reviewing the submitted documents MUST BE SENT ASAP
to prevent any appointment conflicts.
CONTACT LENS WEAR AND FCI/MFS PHYSICAL EXAM APPOINTMENTS
Cadets and pilot candidates coming to their FC I/MFS exams MUST COMPLY WITH THE CONTACT LENS WEAR INSTRUCTIONS:
· Soft contacts lenses MUST
be removed 30 days prior
to any scheduled appointment.
· Hard contacts lenses MUST
be removed 90 days prior
to any scheduled appointment.
: Cadets must be counseled on complying with these instructions. If they are found having worn contact lenses in the 30 (or 90) days prior to their appointment, they will have to repeat a cycloplegic/eyes dilated examination after returning home and forward the results before our FCI physical examination process can continue.
Frequently Asked Questions (FAQs)
Question 1: What is the difference between a FCI done at Medical Flight Standards Branch and another USAF physical exams facility (other than USAFA Cadet Clinic)?
MFS Branch combines all FCI physical exam requirements by AFI 48-123, para 3.1.3 WITH tests specific to Medical Flight Screening evaluations ONLY DONE AT USAFSAM and mandatory prior to USAF pilot training (IFS/SUPT) as defined separately in AFI 48-123, para 6.23.
FC I/MFS Pilot Selection Exams
Question 2: Who do I contact for scheduling of USAF pilot candidate FCI Exams per AFI 48-123 and current policy?
· AFROTC Cadets: please contact your Det NCO for guidance.
· Non-AFROTC University: Please contact your nearest USAF recruiter to help initiate the FCI physical, at an active duty USAF base nearest you, as part of your pilot application/accession package.
· USAF Active Duty applicants: - SCHEDULE WITH YOUR LOCAL ACTIVE DUTY MTF - FORCE HEALTH MANAGEMENT OFFICE. Active duty or non-AFROTC college/university FCI applicants are responsible to get their FCI physicals accomplished at the nearest AFB/current base of assignment Force Health Management/Flight Medicine office.
· Air National Guard:
Question 3: How long will my FCI exam take?
Answer: Your FCI exam is scheduled for 5 days with you present. Under normal circumstances, you'll be done with your exam in 3-4 days; however, every exam is different. We ask you to be flexible and plan on a 5 day TDY. We are not responsible for flight arrangements, nor do we expedite exams based on flight arrangements.
LOST FAA MEDICAL CERTIFICATES
Question 4: I lost my FAA medical certificate that was done in conjunction with my FCI exam; how can I obtain a copy?
(FAA Exams are done at USAFSAM in conjunction with all FCI pilot candidate examinations unless you arrived with a copy of your FAA medical certificate from a previous requirement.)
Answer: You must include a check or money order for $2.00 made payable to the FAA mailed to the address below. The Aerospace Medical Certification Division can FAX you a record of your certificate that is valid for no more than 60 days, which should be enough time to receive your replacement certificate. To receive this fax and order a duplicate certificate, call (405)954-7674 to reach the duplicate certificate desk.

Question 5: Since I had my USAF FCI physical accomplished at a location other than USAFSAM or USAFA - Who do I contact for scheduling of my MFS Only evaluation prior to UPT?

Question 6: How is my "MFS Only" lodging reservation made?
Answer: Automatically, when HQ AFPC, HQ ANG, HQ AFRC schedules you for MFS. In other words, if you've received official notification of your MFS evaluation through the normal scheduling process described above, your room reservation is taken care of. The reservations start the day prior to your appointment with arrival by 1800 and check out is for the day after your appointment. Make departure flight arrangements after 1400 hrs on the Thursday which is the day after your appointment date.
You can call the Wright Patterson Inn 72 hours prior to arrival to confirm the reservation and find out if you are billeted on-base or at off-base contract quarters at (937) 257-3451 or (937) 257-5442, DSN: 787-3451/787-5442. If you need different arrangements, such as late check in or out, you must coordinate directly with the billeting office.
Question 7: When are "MFS Only" evaluations scheduled in the flying training process?
Answer: When any FCI exam is accomplished from a USAF facility other than USAFSAM or USAFA, MFS by AFI 48-123 is still always accomplished prior to IFS/SUPT or RFS and RPA Pilot Training.
Question 8: Once I receive my training RIP to report to Medical Flight Screening, where can I find other reporting information?
Answer: CONTACT OFFICER ACCESSIONS FOR ACTIVE DUTY OR NGB OR AFRC CONTACTS LISTED ON THIS PAGE FOR FURTHER INSTRUCTIONS.
