Initial Flying Class II (Flight Surgeon) examinations

If you are scheduled for your flight surgeon applicant physical with us, please scroll down to the "Begin" below to complete and forward your pre-screening package to us via e-mail org box.

Once scheduled, we will accomplish an in-depth medical examination. Our goal is to provide you with a complete, efficient, and streamlined examination process; many times we are able to complete this in 2 days. Some examinations depending on medical history may require additional sub-specialty evaluations; however, MFS staff members are most often able to coordinate these to completion within 24 hours, so you are able to depart within 3 or 4 days with a sense that your examination has enough information to be forwarded to HQ AETC/SGPS for certification/aeromedical waiver.

How To Schedule
Civilian accessions to active duty: AFPC/DPAMP (flight surgeon accessions) will contact you for times and orders. Next exam times for FY2015 are being established. Contact your local recruiter office.

Civilian accessions to AFRC: Scheduled through HQ AFRC/SGP. Exams will be Monday - Friday concurrent with FCI (pilot) exams. HQ AFRC/SGPA will screen you medical records and work with you on schedule and orders. POC: (478)327-0941 DSN: 497-0941.

● Currently, all other flight surgeon applicants (active duty physicians, ANG) must go through their local Flight Surgeons/Force Health Management Office at their military medical treatment facility.

Pre-exam Processing

There are a few administrative steps you must complete prior to your FCII physical exam appointment. All these pre-screening requirements should be accomplished as soon as possible, prior to your appointment to avoid last minute medical records requests which could result in rescheduling or cancellation of your appointment. We require our pilot candidates submit this package 30-45 days prior depending on their medical history. As you proceed, please read all the information carefully and make sure you have completed everything before moving on to the next step. This will ensure you do not miss any requirements.
