CIVILIAN DEPENDENT ID CARDS

CIVILIAN EMPLOYEES CAC Identification Card
RETIRED CIVILIAN EMPLOYEES Retired Identification Card

SPOUSE
Renewals Unexpired Civilian Dependent ID

New Card Issue Photo Identification AND Certified Marriage Certificate

WIDOW/WIDOWER:   Photo Identification AND Certified Marriage Certificate AND Death Certificate of former spouse AND; Retirement SF-50 of former spouse OR proof of annuity receipt by widow/widower

DEPENDENT CHILD
10 - 18 years old: Certified Birth Certificate AND Photo ID (if available)

19-21 years (unmarried student): Proof of Full Time Enrollment in an accredited Institution  of Higher Learning (e.g., letter from the registrar's office
certifying full time enrollment;) AND Certified Birth Certificate; AND Photo ID or Passport or State ID

Disabled child 21 years old and above:  Appropriate documentation to verify disability or  incapacitation AND Certified Birth certificate AND Photo ID (if available). 

Social Security Numbers will be required for employees or retirees and for all dependents to complete the Application for Identification Card request

Civilian Dependent ID cards CANNOT be issued, reissued or renewed for:
1. Employees of other Federal agencies or military departments, unless the wing commander (or equivalent) authorizes it
2. Private sector contractor personnel who work or perform services for the Air Force
3. Other non-Air Force retired civilian employees and family members
